Become an Agent

Thank you for your interest in our Independent Agent program.

The following link will take you to the state's website for you to apply for your 02-52 Service Warranty Representative license.

Make the following selections:

  • For an individual application, choose the "Individual" tab on the left
  • Scroll down to the bottom of the page and enter your SS# and DOB
  • On the following page, choose "Initial Application for Agent License"
  • For a Firm, choose the Business / Tile tab on the left
  • Scroll down to the bottom of the page and enter your FEIN#
  • On the following page, select "Initial Application for Firm License"

Answer all demographic and background questions.

Type of License; 02-52 Service Warranty Sales Representative

The application fee is $55.00, plus a small processing fee.

The approval from the state will take approximately 2 weeks.

Once you have obtained your License Number, you will need to be appointed.

You will need to provide the following information:

  1. Your Full Name
  2. License Number issued
  3. Name of Firm, Address, Phone Number and Fax Number

In addition, the state charges $60, plus a small processing fee for your appointment. Therefore, you will also need to provide us with a credit card, your name as it appears on the card, billing address, and the expiration date.

The appointment process takes approximately 24 - 48 hours. We will send you confirmation via e-mail as soon as we receive notification from the State that the process is complete.

Lastly, you are required to provide us with a completed W-9.
